Establishing care with us is pretty easy.

We love offering really personalized care and, just like any good relationship, that takes effort from both of us.

Help us get to know you by telling us a little bit about yourself. You can do this by filling out one of the following forms. Yeah, we know, “ugh, forms.” Hopefully these ones are a little different. We do this so we know your backstory before you even walk through the doors. That helps us to focus on what is important to you and to be sure we are prepared to coordinate the care you deserve.

We will review everything you include and call you within three business days (but often much sooner) with next steps. Midwives, naturopathic doctors, and the birth center model of care are effective, evidence-based choices for most people, but may not the best fit for everyone. We can help you understand all your options so you can choose what is best for you.

There are some situations in which we are unable to provide your care, but we will let you know if and why that is the case.

Baseline, we want you to be safe, seen, and cared for. We want what is best for you, whatever that looks like.
